New Patients

YOUR FIRST APPOINTMENT WITH US

In order for our doctors to conduct a complete eye exam with dilation and thoroughly explain any findings from the visit, please allow 90 minutes for the first appointment. We suggest that you have snacks and baby's necessities on hand. If you or your child's eyes are dilated, they will be sensitive to light. We suggest that you bring sunglasses for you or your child to wear after your visit. The eye drops will make your or your child's near vision blurry. Depending on the drops that are given, the dilation can last anywhere from 4 to 24 hours.

IF ANYONE OTHER THAN THE PARENT OR LEGAL GUARDIAN IS ACCOMPANYING THE CHILD, THEY MUST BRING WRITTEN PERMISSION FROM A PARENT OR LEGAL GUARDIAN IN ORDER FOR THE PATIENT TO RECEIVE TREATMENT

Insurance Information

Dankner Fiergang Eye Associates participates with or accepts many different insurance companies, including many traditional plans and HMO's. Please inquire about your specific plan when you call our office for an appointment.

As we are familiar with most insurance company plans, we ask that you have your current insurance information available when calling our office to schedule an appointment so as to help us determine your specific insurance plan requirements.

Most insurance companies now require preauthorized referrals for your visit. In order for us to bill for our services on your behalf, please bring your most current insurance card and preauthorized referral with you on the day of your visit. Otherwise, you may be asked to pay for your visit in full.

For your convenience, we accept many forms of payment. Co-pays are required to be paid at the time of the visit. Dankner Fiergang Eye Associates accepts cash, personal checks (subject to a NSF fee), Visa and MasterCard credit cards, and Visa and MasterCard debit cards.

We want to be of service to you and your family by providing you with assistance in answering your payment and insurance questions. If you have any questions concerning your coverage, contact us and we will be happy to assist you prior to your examination.

After your visit and where appropriate, we will bill your insurance company on your behalf and will provide them with the necessary information they require in order for Dankner Fiergang Eye Associates to be reimbursed for our services. Occasionally and despite our best efforts, your insurance company may elect not to reimburse us on your behalf. In those rare situations, we may ask for your assistance to help us resolve the matter
